The root cause is rarely apparent. A normal reaction is to jump to a conclusion based on an assumption or so-called “experience”. This leads to addressing the wrong problem or, even worse, blaming the wrong persons. While it is important to initiate an interim containment plan as soon as a problem occurs, it is even more important to invest the time and energy to identify and address the root cause or causes of those problems. This is the only way to instill countermeasures to avoid future reoccurrence.
